Large collections in multiple spaces, free-floating bowel
Position: Probe marker cephalad. Identify liver-kidney interface (Morison's pouch). Fan through the space.

Clinical Pearls
  • Fan superiorly to assess the subphrenic space — often missed
  • Fluid in Morison's pouch tracks from the pelvis — always check pelvis too
  • Hepatic veins entering the IVC confirm correct depth
